Activity Description

Pain at the end of life remains frequently under-recognized and undertreated, representing a critical gap in palliative and hospice care. This activity provides clinicians with a comprehensive review of the pathophysiology, assessment, and interdisciplinary management of pain in terminally ill patients. Participants will examine the components of total pain, including physical, psychological, interpersonal, and existential dimensions, and explore evidence-based pharmacological and nonpharmacological strategies. The activity addresses deficiencies in pain assessment, opioid selection and titration, recognition of adverse effects, and goals-of-care communication. Clinicians will gain actionable skills to individualize treatment plans, prevent complications, and engage patients and families in shared decision-making. Emphasis is placed on strengthening interdisciplinary collaboration to optimize comfort, preserve dignity, and improve patient-centered outcomes at the end of life.


Target Audience

This activity has been designed to meet the educational needs of physicians, physician associates, nurses, pharmacists, nurse practitioners, dentists, and social workers.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Differentiate among somatic, visceral, and neuropathic pain based on clinical presentation to guide the selection of analgesic therapy.

  • Evaluate the adverse effect profiles of analgesic agents, including opioid-induced hyperalgesia, respiratory depression, gastrointestinal tract complications, and hepatic and renal toxicity.

  • Determine appropriate goals-of-care documentation strategies, including advance directives and medical orders for life-sustaining treatment, to ensure adherence to patient wishes and prevent unnecessary interventions.

  • Collaborate with interdisciplinary team members, including palliative medicine specialists, nurses, chaplains, social workers, wound care clinicians, and respiratory therapists, to coordinate comprehensive, patient-centered pain management at the end of life.

I think this assessment needs reviewed. answer to #1--- in an elderly patient with decreased oral intake of fluids, high fiber in the diet can make constipation worse. #5--there should be consideration to the placement of a venting G tube, this is pretty standard offering with data that this improves quality of life and decreases days in hospital.
