Activity Description

Lateral patellar compression syndrome (LPCS), also called excessive lateral pressure syndrome, represents a common but under-recognized cause of anterior knee pain in the absence of patellar instability. The condition is marked by excessive pressure on the lateral patellofemoral joint due to a tight lateral retinaculum, which restricts normal medial glide and induces lateral patellar tilt. This biomechanical imbalance disrupts patellar tracking within the trochlear groove, increasing joint stress and leading to chondropathy of the lateral facet. Without timely intervention, LPCS can progress to patellofemoral osteoarthritis due to chronic cartilage overload. Early identification and appropriate treatment are essential to prevent long-term degenerative changes.

This educational activity enables the participant to gain a thorough understanding of the pathophysiology, epidemiology, clinical presentation, diagnostic approach, and management strategies for LPCS. The course emphasizes the importance of early recognition to prevent cartilage degeneration and long-term joint dysfunction. Participants learn to distinguish LPCS from patellar instability and other causes of anterior knee pain, allowing for targeted, patient-specific interventions. Conservative strategies such as physical therapy, patellar taping, and stretching of the lateral retinaculum are reviewed, alongside indications for surgical release in refractory cases. Collaboration with an interprofessional team—including physical therapists, radiologists, orthopedic clinicians, and sports medicine specialists—enhances diagnostic accuracy and optimizes individualized treatment plans. This team-based approach supports improved functional outcomes, reduces the risk of progression to osteoarthritis, and promotes patient-centered care.

Target Audience

This activity has been designed to meet the educational needs of podiatrists, podiatry residents, and other medical professionals treating conditions.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify the clinical signs and symptoms of lateral patellar compression syndrome to distinguish it from other causes of anterior knee pain.

  • Assess the clinical features and investigation findings required to make the diagnosis of lateral patellar compression syndrome.

  • Select the appropriate management option in the treatment of lateral patellar compression syndrome.

  • Improve communication and coordination among the interprofessional healthcare team, including physical therapists, orthopedic clinicians, and radiologists, to enhance diagnostic accuracy and optimize patient-centered management.
