Activity Description

Acute vision loss, ocular trauma, headache, and suspected intracranial hypertension are time-sensitive emergency presentations. Etiologies include retinal detachment, vitreous hemorrhage, central retinal artery occlusion, lens dislocation, retrobulbar hematoma, intraocular foreign bodies, and elevated intracranial pressure. Risk factors include trauma, degenerative vitreous changes, vascular disease, diabetes mellitus, and connective tissue disorders. Patients may present with photopsias, floaters, scotomas, sudden painless vision loss, visual field deficits, orbital pain, or neurologic symptoms. Examination is often limited by swelling, corneal opacity, or poor visualization. Point-of-care ocular ultrasound enables rapid bedside evaluation when direct visualization is inadequate. Indications include suspected retinal or vitreous pathology, lens dislocation, intraocular foreign body, retrobulbar hematoma, and elevated intracranial pressure; suspected globe rupture is a contraindication. When performed by trained clinicians, diagnostic accuracy is high. Mastery of technique and interpretation supports timely diagnosis, clinical decision-making, and interprofessional care.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Evaluate patients for suitability for ocular ultrasound based on clinical presentation and risk factors.

  • Interpret point-of-care ultrasound findings to diagnose acute vision-threatening pathology.

  • Determine patient suitability for urgent escalation of care after ocular ultrasound based on identified pathologic features and clinical context.

  • Collaborate with members of the interprofessional team, including specialists such as emergency medicine physicians, ophthalmologists, and radiologists, to provide efficient, comprehensive, and coordinated care for individuals undergoing ocular ultrasound.
