Activity Description

Venous gas embolism (VGE) is the accumulation of gas within the systemic venous circulation, forming embolic bubbles that disrupt hemodynamics. Approximately 90% of cases are iatrogenic, occurring during medical or surgical procedures, particularly when wounds are located above the phlebostatic axis. VGE's pathogenesis requires a gas source, a patent connection to the venous system, and a pressure gradient promoting air entry. Large boluses of air can create an "air lock," impairing right ventricular ejection, while smaller volumes lodge in pulmonary arterioles, causing pulmonary arterial hypertension and acute right ventricular failure. The clinical presentation ranges from dyspnea, chest pain, hypotension, and hypoxemia to neurological deficits in paradoxical embolism, with severe cases resulting in death.

Diagnosis relies on clinical suspicion, supported by transesophageal echocardiography, precordial Doppler, or computed tomography in selected cases. Immediate management includes positioning in left lateral decubitus and Trendelenburg, high-flow oxygen administration, and hemodynamic support. Hyperbaric oxygen therapy is indicated in severe cases. Complications include pulmonary edema, coagulopathy, and arterial embolism. Prognosis depends on volume and rate of air entry, prompt recognition, and timely intervention, with rapid management improving survival and limiting end-organ injury.

Participants will deepen their understanding of the condition's causes, risk factors, pathophysiology, clinical presentation, and recommended diagnostic and treatment approaches. Mastery of these concepts will optimize interprofessional care for patients experiencing VGE.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Differentiate venous gas embolism from other causes of acute cardiovascular and respiratory compromise, including pulmonary thromboembolism, myocardial infarction, and septic shock, based on pathophysiology, clinical presentation, and diagnostic findings.

  • Develop a diagnostic strategy to confirm venous gas embolism, grade the condition's severity, and monitor for complications.

  • Implement evidence-based, personalized approaches for managing venous gas embolism and mitigating its potential sequelae.

  • Collaborate with all members of the interprofessional team, including surgeons, internists, and hyperbaric medicine specialists, to provide timely, coordinated, and guideline-directed management of venous gas embolism.
