Activity Description

Human cytomegalovirus, also known as herpesvirus 5, is a ubiquitous virus that belongs to the Herpesviridae family, a large group of DNA viruses. Cytomegalovirus has the largest genome of any herpesvirus and has a worldwide seroprevalence of 24% to 100%. As with other herpesviruses, it establishes lifelong latency. Primary cytomegalovirus infection can manifest as either an asymptomatic infection, a flu-like syndrome, mononucleosis, or, in rare cases, other, more serious organ-specific syndromes. Following cytomegalovirus exposure, latency can allow the virus to persist in host cells. In immunocompromised patients, such as those who have undergone solid organ and hematopoietic stem cell transplantation, or those undergoing chemotherapy, cytomegalovirus can reactivate, leading to cytomegalovirus DNAemia. This can cause severe organ damage, including cytomegalovirus esophagitis and colitis. Reactivation of cytomegalovirus is associated with high morbidity and mortality and can cause rejection of transplanted organs. Cytomegalovirus can also be transmitted to the embryo in mothers with cytomegalovirus infection, potentially causing congenital cytomegalovirus. This activity reviews the role that healthcare professionals play in evaluating and treating patients with cytomegalovirus infection, empowering healthcare providers with the knowledge to care for individuals affected by this condition.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify the epidemiology of cytomegalovirus infections, with a focus on immunosuppressed populations, including patients with HIV/AIDS and transplant recipients.

  • Assess patients who are at risk for cytomegalovirus infection and those who exhibit symptoms consistent with this condition.

  • Implement evidence-based therapeutic interventions for patients with cytomegalovirus infections.

  • Collaborate with interprofessional teams, including infectious disease specialists, gastroenterologists, pharmacists, and transplant teams, for comprehensive care.
