Activity Description

Bupivacaine is an amide-type local anesthetic widely used in clinical practice for regional anesthesia, including epidural, spinal, caudal, and peripheral nerve blocks, as well as infiltration anesthesia. The mechanism of action of the drug involves reversible blockade of voltage-gated sodium channels, thereby inhibiting nerve impulse conduction and producing prolonged anesthesia and analgesia. Bupivacaine is particularly useful for procedures requiring extended sensory and motor blockade, including surgical anesthesia, obstetric anesthesia, and postoperative pain management. This activity provides a comprehensive review of bupivacaine’s pharmacology, clinical indications, contraindications, adverse effects, drug interactions, and key monitoring parameters. This activity also develops an understanding of appropriate patient selection, individualized dosing strategies, and methods to minimize toxicity, particularly in patients with hepatic or cardiovascular comorbidities. In addition, this activity also emphasizes interprofessional collaboration, focusing on patient counseling and adherence to evidence-based guidelines to enhance safety, maximize therapeutic efficacy, and reduce preventable complications associated with bupivacaine administration.

This activity has been designed to meet the educational needs of physicians, physician associates, nurses, pharmacists, nurse practitioners, and dentists.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify the indications, contraindications, pharmacologic properties, and adverse effect profile of bupivacaine used in regional anesthesia and analgesia.

  • Implement evidence-based monitoring and safety strategies during and after bupivacaine administration to reduce preventable complications.

  • Select appropriate rescue interventions, including lipid emulsion therapy, for the management of suspected local anesthetic systemic toxicity.

  • Collaborate with interprofessional healthcare providers to ensure safe bupivacaine administration and emergency preparedness, including access to resuscitation equipment and lipid emulsion therapy.
